Provider FAQ

  • Fill out and submit a Provider Registration.

  • Sample collection kits, containers, and biohazard bags are provided by FutureGeneration Laboratories. Contact client services with the name of your facility and quantity of supplies needed.

    Test requisitions can also be downloaded here. A test requisition can also be autogenerated by creating an electronic order through the provider portal.

  • Courier services are provided by FutureGeneration Laboratories and its partners.

    Contact client services with the name of your facility and quantity of specimen for pick up.

    1. Verify that the clinical sample is: tightly capped to prevent leakage; labeled with at least 2 patient identifiers (full name and date of birth); the label is not blocking the window to view specimen quantity; urine specimen: liquid level is within the black lines; swab specimen: correct swab is used and swab is broken at the score line

    2. Place the clinical sample in a puncture-proof and leak-proof biohazard bag.

    3. Place a complete test requisition on the outside pouch of the biohazard bag.

    4. Specimen may be stored at 2°C - 8°C for up to 3 days prior to transport to laboratory.

  • Follow the instructions for creating an electronic order through the provider portal.

  • The patient’s identity should be verified using TWO separate identifiers. The patient’s full name and either date of birth or social security number will be verified. The collector should ask the patient to state their full name, correct spelling and date of birth or social security number.

    The collector should also verify the correct information on the test requisition/physician order:

    • Name of ordering physician

    • Address of ordering physician

    • Name of patient and second identification

    • Tests to be completed

    • Diagnosis code

    • Other clinical information necessary to process the test request

  • Upon completion of testing, FutureGeneration Laboratories will retain the sample at the appropriate storage temperature for up to 7 calendar days.

  • FutureGeneration Laboratories accepts request for additional testing to be performed on specimens already received in the laboratory. The laboratories accept add on requests only on those analyses that maintain their integrity during storage. Addition of tests depends on the stability of the analyses.

  • Tests may be cancelled without chart while it is in transit. To cancel a test, contact Client Services. If a test has been assayed, cancellation will not be accepted and the test will be billed.
